Before penicillin was isolated from mold in 1928, nature had already invented antibiotics. Plants, bees, and bulbs have lived among bacteria for millions of years, and they developed to survive chemical defenses of an effectiveness that modern research measures today with its own tools.

What is called natural antibiotic, and how it is measured
A natural antibiotic is a substance that a plant, insect, or microorganism produces to defend itself against bacteria. The hive is a warm and humid environment where everything would rot without propolis. Nigella seeds must resist soil molds. Garlic only produces allicin when it is damaged, as an emergency response. These molecules are the result of natural selection against microbes, and this is what makes them valuable for research.
Studies measure their activity in three complementary ways, and this article clarifies for each product what has been done.

A synthetic antibiotic most often acts on a single target, which a single bacterial mutation can bypass : this is the mechanism of resistance. Honey, propolis, or nigella act through multiple simultaneous mechanisms, osmotic, enzymatic, membranous, which makes bacterial adaptation much more difficult. The trial on propolis in dialysis, presented below, states this explicitly : the local antibiotic can induce resistance, propolis does not.
This is one of the reasons why these ancient substances interest current research so much, at a time when antibiotic resistance is becoming a major public health problem.
Honey : wounds and respiratory tract
Honey is antibacterial through a combination of mechanisms that few substances possess : an osmolarity so high that it dehydrates bacteria, an acidic pH unfavorable to their growth, the continuous production of hydrogen peroxide by an enzyme that bees add to it, and for manuka honey the methylglyoxal, a powerful antibacterial compound.
On wounds: the Cochrane review
The Cochrane Collaboration, the global reference for treatment evaluation, compiled 26 trials and 3,011 participants on honey applied topically to wounds.

The trial against triple therapy
Helicobacter pylori is the bacterium responsible for most ulcers and many cases of gastritis. Its standard treatment is a triple therapy combining two antibiotics and an acid inhibitor. A randomized trial in 88 patients infected compared this triple therapy to three doses of black seed associated with omeprazole, for four weeks.

Digestive symptoms improved similarly in all groups. The authors conclude that black seeds possess anti-Helicobacter activity clinically useful, comparable to triple therapy, and call for trials combining them with antibiotics to enhance their effect.
It is rare for a food seed to be directly compared against a standard antibiotic protocol for bacterial eradication, and to hold its own in the comparison. At 2 g per day, approximately one teaspoon of ground seeds, black seed eradicated the bacterium in two out of three patients. This is the strongest result in this article, and it concerns a real bacterial infection, not an in vitro test.
The intermediate dose performed better than the high dose, suggesting that beyond 2 g the benefit does not increase. This type of bell curve is found with other natural substances.
Garlic and black garlic: colds and immunity
Garlic producesallicin at the moment it is cut or crushed, through an enzymatic defense reaction. It is a broad-spectrum antimicrobial, active in the laboratory against bacteria, fungi, and certain viruses.
The cold trial
146 volunteers received, in double-blind fashion, a daily capsule of garlic rich in allicin or a placebo for twelve weeks, from November to February, noting their status each day.

Participants taking garlic were less likely to catch a cold, and recovered faster when they did catch one. Those on placebo were far more likely to experience multiple colds over the season.
Black garlic: the aged form
Blackgarlic is garlic aged through slow maturation at controlled temperature and humidity, which converts allicin, unstable and odorous, into stable sulfur compounds stable such as S-allyl-cysteine, and which eliminates the odor and digestive irritation. This is the basis of aged garlic extracts studied in clinical trials.
A randomized, double-blind trial in 120 healthy people tested 2.56 g per day of aged garlic extract for 90 days.
