Hyaluronic acid is everywhere: in serums, in creams, in capsules, in injections. The question that always comes up is the same: should you apply it or take it? It's a false choice, because the two don't do the same thing. They don't work in the same place, not at the same speed, and the studies show this with precision.
This article follows the molecule through its two levels. On the surface, what a serum does to the stratum corneum, measured by successive skin sampling, and the effect of molecular weight on wrinkles. Deep down, what the oral route changes in twelve weeks on hydration, elasticity and wrinkles, and why it works so well with collagen. The result is simple: two tools, two levels, and a reason to combine them.
In serum. Hyaluronic acid penetrates and accumulates in the stratum corneum: 8 times more 24 hours after application, 31 times more after seven days. In 76 women, all molecular weights tested improved hydration and elasticity in 60 days, and low molecular weights, 50 and 130 kilodaltons, additionally reduced wrinkle depth.
In capsules. Ingested hyaluronic acid is absorbed, distributed to the skin and stimulates fibroblasts. At 120 mg per day for 12 weeks, in 40 adults in a double-blind study, it improved wrinkles, hydration, water loss and elasticity. With collagen. Collagen peptides stimulate the skin's production of hyaluronic acid: the two reinforce each other. The conclusion: the serum works on the surface in hours, the capsules on the depth in weeks. Skin has two layers that matter here. The stratum corneum, on the surface, where hydration is measured in hours and is visible to the naked eye. The dermis, in depth, where fibroblasts live and produce collagen and hyaluronic acid, and where long-term firmness is determined.

In serum: what reaches the stratum corneum
To determine whether a hyaluronic acid serum does more than stay on the surface, a cosmetic research team used the most direct method: tape stripping, which removes the stratum corneum layer by layer, then measures hyaluronic acid content in each layer.
Volunteers applied daily for seven days a formulation containing high and low molecular weight hyaluronic acid. The stratum corneum was sampled before, then two hours after application, on days 0, 1, and 7, using two cleansing methods to ensure they weren't measuring simple residual film.

The authors conclude that diffusion, accumulation, and retention of hyaluronic acid in the stratum corneum occurs after repeated application. The serum doesn't just glide across the skin: it charges it, day after day, and the charge persists.
It establishes that hyaluronic acid from a serum penetrates the stratum corneum and accumulates there, which is precisely what it's asked to do: this layer is the skin's surface water reservoir, the one that makes skin supple or tight, smooth or wrinkled. This is where visible hydration happens.
The method used measured the stratum corneum; it wasn't suited for deeper layers. What the serum does to the dermis is therefore not measured here, and that is precisely the territory of the oral route, in section 4. This study came from a cosmetic laboratory, which we are noting.
Molecular weight: the difference between hydrating and smoothing
Not all hyaluronic acids are alike. The molecule exists in chains of very different lengths, measured in kilodaltons, and length determines what it can do. A dermatology team from Munich tested this question in the cleanest way: in 76 women ages 30 to 60 with wrinkles around the eyes, a cream containing 0.1% hyaluronic acid with five molecular weights, 50, 130, 300, 800, and 2,000 kilodaltons, applied twice daily for 60 days around one eye, with a placebo cream around the other.

The results are clear. All molecular weights improved hydration and elasticity compared to placebo: on this point, hyaluronic acid in serum works regardless of its size. But only the smallest, 50 and 130 kilodaltons, reduced wrinkle depth, measured by skin impressions. The authors attribute this difference to better penetration of shorter chains.
In capsule form: what oral administration changes in twelve weeks
Swallowing hyaluronic acid for skin seems counterintuitive: the molecule is large, and one wonders how it reaches the face. A literature review published in Nutrition Journal summarizes the established chain of mechanisms: ingested hyaluronic acid is absorbed by the intestine, distributed in part to the skin, where it stimulates hyaluronic acid synthesis by fibroblasts and promotes their proliferation. It therefore acts not only by providing the molecule, but by restarting its production in the dermis.
The twelve-week trial
A randomized double-blind trial versus placebo, published in Nutrients, followed 40 men and women aged 35 to 64 years taking 120 mg of hyaluronic acid per day for 12 weeks, with comprehensive skin evaluation.

The authors conclude that ingestion of hyaluronic acid can reduce wrinkles and improve skin condition. The remarkable point is transepidermal water loss : this is the measure of skin barrier quality, and an oral route improved it, which indicates a deep effect and not just a surface effect.
The twelve-week trial and the review preceding it include authors affiliated with Kewpie, Japanese manufacturer of hyaluronic acid. This is common in this field, where research is largely funded by ingredient producers. A randomized double-blind trial remains a randomized double-blind trial, and its measurements are objective, but the reader should know where it comes from. The sample size, 40 people, is that of a modest-sized trial.
Why collagen and hyaluronic acid reinforce each other
Collagen and hyaluronic acid are the two major components of the dermis, manufactured by the same cells, fibroblasts. What links them goes further: the collagen peptides absorbed after ingestion of a hydrolysate, in particular the dipeptide prolyl-hydroxyproline, have been shown capable of stimulating fibroblasts andincreasing their hyaluronic acid production.
A randomized double-blind trial compared two collagen hydrolysates, one rich in these active dipeptides, the other poor, against placebo, in women over eight weeks. The hydrolysate rich in active peptides significantly improved, and more than the other and placebo, thehydration,elasticity, wrinkles and roughness of the face, with no adverse effects.

Both, because they don't work in the same place. The serum hydrates the stratum corneum, the skin's surface, and its effect is measured in hours: one study shows that hyaluronic acid accumulates there up to 31 times after one week of application. Capsules pass through the bloodstream to the dermis, and their effect is measured in weeks: a 12-week trial at 120 mg per day improved wrinkles, hydration, transepidermal water loss, and elasticity. One works the surface, the other works the depth.
What is hyaluronic acid?
A molecule naturally present in all connective tissues in the body, of which skin contains about half. Its distinctive feature is retaining a considerable amount of water—up to a thousand times its weight—which gives it its role in skin hydration, volume, and suppleness. Its quantity in the dermis decreases with age, which contributes to dryness and loss of firmness.
Are the studies independent?
Not all of them, and we say so. The 12-week oral trial and the review on ingested hyaluronic acid include authors affiliated with a Japanese hyaluronic acid manufacturer. The skin penetration study comes from a cosmetics laboratory. The trial on molecular weights and the one on collagen peptides also include industrial authors. This is common in this field, where research is largely funded by manufacturers; this doesn't invalidate double-blind trials, but it should be noted.
The Serum
Does hyaluronic acid serum penetrate the skin?
It penetrates and accumulates in the stratum corneum, the outermost layer. One study measured its presence through successive samplings of the stratum corneum in volunteers: 24 hours after the first application, the level was at least 8 times higher than before, and after seven days of daily application, at least 31 times. The molecule diffuses, accumulates, and remains in the stratum corneum. Deeper layers were not measurable by this method.
Does the molecular weight of hyaluronic acid matter?
Yes, and it's measured. In 76 women aged 30 to 60, creams containing 0.1% hyaluronic acid at five different molecular weights, from 50 to 2,000 kilodaltons, were applied around the eye for 60 days against placebo. All molecular weights improved hydration and elasticity. But only the smallest ones, 50 and 130 kilodaltons, significantly reduced wrinkle depth, probably because they penetrate better. A low molecular weight serum therefore does more than just hydrate.
Is serum enough for wrinkles?
It acts on surface and dehydration wrinkles, and the molecular weight trial shows measurable reduction in wrinkle depth around the eye with low molecular weight hyaluronic acid. Deeper wrinkles depend on the dermis, which the serum doesn't reach; that's the domain of oral intake. For complete results, both levels work together.
How to apply a hyaluronic acid serum?
On clean and slightly damp skin, morning and evening, before moisturizer. Hyaluronic acid captures water: applying it to damp skin gives it water to retain, and covering it with a cream prevents it from releasing that water to the ambient air in dry conditions. The trial on 76 women used two applications per day for 60 days.

Is oral hyaluronic acid absorbed?
Yes. A literature review summarizes the chain of mechanisms: ingested hyaluronic acid is absorbed by the intestine, partly distributed to the skin, where it stimulates hyaluronic acid synthesis by fibroblasts and their proliferation. This explains why oral intake can act on the dermis, where a serum cannot.
What does the 12-week trial on oral hyaluronic acid show?
In 40 men and women aged 35 to 64, in a double-blind trial against placebo, 120 mg of hyaluronic acid per day for 12 weeks significantly improved wrinkle assessment, water content of the stratum corneum, transepidermal water loss, and elasticity compared to placebo. Improvements in wrinkles, hydration, and elasticity were already significant at 8 weeks. Several authors are affiliated with the ingredient manufacturer, which we note.
What oral dose was studied?
120 mg per day in the 12-week trial, which is the most studied dose in the skin literature. It's a useful reference for reading a label: a capsule that contains significantly less does not reproduce what was tested.
When to take hyaluronic acid in capsules?
At any time of day, with or without meals, in one regular daily dose. The 12-week trial used one daily dose. Consistency matters more than timing, since the effect builds over weeks.
Why combine collagen and hyaluronic acid?
Because they stimulate each other mutually. Absorbed collagen peptides, particularly the prolyl-hydroxyproline dipeptide, stimulate fibroblasts and increase their hyaluronic acid production. A randomized trial in women showed that a collagen hydrolysate rich in these dipeptides improved hydration, elasticity, wrinkles, and skin roughness in 8 weeks. Providing both means supplying the material and the signal that makes it produce.
Hyaluronic acid and joints?
Hyaluronic acid is also a component of joint fluid, and it has been studied orally for knee comfort. That's not the subject of this article, which focuses on skin, but it's an additional reason why it is often combined with marine collagen in oral formulas.

How long before seeing an effect?
For serum, surface hydration is measurable within the first few hours and accumulation builds over the first week; the effect on wrinkles takes about two months in the molecular weight trial. For oral intake, the first significant differences appear at 8 weeks and are confirmed at 12. Both are gestures of consistency, not immediacy.
Can serum and capsules be used at the same time?
Yes, and that's the logic of this article. The serum works the stratum corneum and delivers visible results quickly; capsules work the dermis and deliver results that develop over time. They don't compete since they don't work at the same level. People who want an immediate surface effect and a lasting deep effect benefit from combining them.
Is hyaluronic acid suitable for all skin types?
In topical use, it is one of the best-tolerated active ingredients in cosmetics, including on sensitive skin, and the 76-woman trial reported no irritation. By oral route, the 12-week trial reported no adverse effects. People allergic to any of the other components of a product should of course read the ingredient list.
